I always start from a cold cockpit in a parking slot. I like starting her up and warming up the engines and then taxiing out to the runway. It's those details that makes the sim realistic in my opinion. Only time I start from a runway with the engines going is when I'm just testing something out and don't want to take the time for a whole checklist.

I always start from where I left off the night before. Whether it's midflight or I'm on the ground. I'm just on one continuous flight from one place to another. I go through the motions of refueling and parking to keep it real. Right now my game is saved and I'm somewhere between Dallas and Denver. ;)

A parking stall if available.  If there isn't one then I use afcad to add a parking spot.  So in short a parking spot....  But in reality my flight begins on my drive home from work.

(while driving)"Alright, I have to take care of the dogs, do some dishes, do this, do that, then I can start my flight from a parking spot at_____ and fly to ____, or maybe ______......."

On the Runway - Not that I am lazy but I still haven't mastered the taxi - ing everytime I try I end up crashing into a terminal or another plane or getting lost trying to find the right runway - At least if I start on a runway I can get up in the air then find a nice airport that I can crash at ! By the way I warn my passengers beforehand that their lives are in danger!

You should request "Progressive Taxi Instructions" from ATC and they will draw a nice big pink line on the ground for you to follow...that way you will get your passengers there is one piece!  ;D
