Alex, the fxh file is in ASCII format, and I suspect that you're creating a new file with notepad, or editing it. Don't do that.
Download Ingvarr's Water20.fxh, and copy and paste that file into your ShadersHLSL/Terrain folder. Don't copy text, only paste the entire file, and it should work.
Basically, Flight Simulator X only reads the Shaders files if they are formatted in 'ASCII', which isn't default in Notepad. Ingvarr's work's because it is an edited original, which means it kept the same format. If you create your own .fxh file with Notepad, it will save as Unicode, which is why it isn't read.
When my program comes out, it should be able to mend your problem. I might send you an advanced copy if you really need it, but remember, it's still beta.
