Due to a number of factors, such as the plane's de-ice being off, flight 90 crashed shortly after take off at Washington National airport. It crashed into the Potomac river after hitting a bridge. 78 people died.

Had to briefly study that crash in my Aviation Meteorology class.. Very unfortuneate crash. Could have easily been avoided too.
