Not dramatic or anything, but this might interest some: a regular poster on the Pilots of America board recently had a little malfunction at a not-very-good moment during initial approach to Hayward, CA... he did a great job of just quickly surmising what was wrong then letting ATC know right away. Turned out very well.
Here's an audio clip; his call sign ends in "1N". He's very scratchy at first, but you can understand him better at about the time he reports the problem.
