This may help......




However, this is no biggy. It was just the outer pane. However, the shock factor for the passengers due and for the pilot sat in front of it are a bigger danger due to over reaction.

As Hagar said. Short version, it was caused by the technician not using the AMM and taking the wrong screws. They were too short and the rest is history
