Not knowing what make and model system you have makes diagnostics a bit harder. I've seen this once or twice before. On one occasion, the Universal Hub devices needed to be updated. On another occasion the PCI bridge drivers were at fault.
Try reinstalling the software that came with your mother board from the CD (if you have one). Then, immediately update all software from manufacturers for your mainboard. The PCI host bridge can sometimes (manufacturer dependant) change how the USB devices operate. This can be true of a software driven modem in particular.
Everything seems to work, but the USB bridge seems to be faulty. If all your software is working and updated, I would suspect what others have already stated, (bad hard card, dirty or poor connections, etc).
