How to register?

Friend of mine keen to register, but can't find where or how. Nor can I : 'Help' says it should be at the top of the page, but it's not on my screen... ???

Where it says "Hey, xxx, you have xx messages.

When you're not logged in it says "Welcome guest, please login or register."
