However, this is no biggy. It was just the outer pane. However, the shock factor for the passengers due and for the pilot sat in front of it are a bigger danger due to over reaction.

What airline was it that had the pilot's windshield blow out several years ago and the pilot was daggling out of the cockpit? The first officer made the landing.

As Hagar said. Short version, it was caused by the technician not using the AMM and taking the wrong screws. They were too short and the rest is history

Edit. If you search Youtube for the Discovery Channel series Aircraft Investigations, there is an episode that covers this accident. I will see if I can find a link.
