Considering the many who have their head in such position, he has found a lucrative profession if, that is, his patients can find their way to his office.
His sign on the front lawn reads:

Entrance at Rear
For those of us with our heads and mouth where they should be, an extraction in his office would be more than just a pain in the butt.
