She's not even a doctor yet. She just wants to find out why all these old men, ("... particularly among those who already have existing health problems.") drop dead after shoveling away the product of major snow storms.

That card board position in his right hand is what I thought was the give away too. His left hand isn't really correct either as none of his fingers are visible on the poster itself. Even if he was holding it by the edge his fingers would overlap the edge somewhat. Still a funny poster.
