How the hell do you start the default FSX 747-400 without pressing Ctrl + E?

::)ALL plane-engines start by "CTRL+E" if FSX-config is default!
Maybe You try to start by moving any switches at panels---also working if
the start-procedure is correct (see =>plane operations)

You need to enable fuel flow beneath the throttle (there 4 switches, turn them up) and then start them using the upper light panel (Ctrl5) there 4 start switches.
Despite this i find all the engine starts in FS a joke. The APU which is a mandatory required ON thing to start engines in real planes is just optional in FSX

It's not really "in FSX". It's just the default starter gauge that does not consider the APU.

Right. I've found the 747 to be one of the worst default aircraft panel-wise. If you want a realistic startup, you're much better off going for addons, probably payware.
