That still depends on what you mean.  "Real" can only go so far on a PC.

If you mean graphics-wise, a good video card and add-on scenery helps.

If you mean operation-wise, follow the "Lessons" from the startup menu.

If you have the money, check out the "Home Built Cockpit" topic.

Other than that, to make it as real as it can get on a PC sim, set your settings on the "realism" page to "realistic" (all slider all the way to the right), in the "Flight Model" box.  In the "Crashes and Damage" box, check the "detect crash and damage", and "aircraft stress causes damage" and "allow collision with other aircraft".  On the "engines" box, uncheck "automixture" and "Unlimited fuel".  Check the box for "G effects" in the "special effects box", an do not enable auto rudder in "flight controls".  Enable "Gyro drift" and "indicated Air Speed" in "Instruments and Lights".  When you start your flights, use ATC for taxi and takeoff instructions, and on approaches, landing and parking.

Beyond that, it's a "state of mind" where you tune out the real world, and put yourself *in* the sim, mentally.

But I'm still not sure I know exactly what you mean.  ;)

Best place to get the answer to that is probably in the "Flight School" topic, and check out the "Sticky" topics above the discussion topics, which contain sources manuals and has a "sim flight training" tutorial.

Try the program FS passengers. It created airlines and gives you points for good flying and penalises you for breaking the rules. I think its great!
