By turning on all the realism settings and disabling things like automixture, autorudder, etc., is it really "As Real as it Gets", or is something still missing to the game?

Even with realism all the way up, it will always be a simulator. In a real 737 you cant just turn off fuel cutoff and turn the start knob for a few seconds, there is an APU and all those advanced systems. It is very real but to learn all those systems for all those airplanes you better spend $10,000 on flight lessons. And who wants to go through the startup checklist to just to fly for fun?

There's a lot missing from the game. High AOA flight model is typically mentioned as a failing point, but there are more. All in all FS9 is probably "as real as it gets" on the PC for commercial aircraft simulation. But there's still a ways to go.
I suspect though that FS9 is a real useful tool to people taking flying lessons, despite the few holes here and there. And my buddy's Dad flies for US Air and he often practices flying his routes in FS9. So all in all I'd say it's a useful piece of software, but no software is perfect.
