How much do they cost and would it be worth getting one?

They cost quite a bit, you might do a google search to find some of the different manufacturers. I would say that if you already had a yoke and pedals, seperate throttles would be a very nice addition to your sim kit, seeing as how it would increase the ease of properly controlling your engines.

I only have a joystick.

If you don't mean a multi-engine throttle setup, check out the Saitek X45. It has a throttle and joystick, both festooned with programmable buttons and hat switches- AND a rudder bar tucked under the throttle handle, which is a boon for taxiing or slipping, not to mention proper control doing aerobatics. The two are connected by a cable, so you only need one USB port. Mine cost me $75 US, and though it doesn't have force-feedback and is not the finest controller out there, it's a lot of product for the money.

I agree that the X-45 is the stick you'll fall in love with.  I've used lots of joysticks, I even had a CH yoke back in the good ol' days of FS98....the X-45 is the best I've used yet.
