Missed approach

I was wondering what is the difference between a missed approached and a published missed approach.  They both seem to do the same thing.

The published missed approach have the word "published" in it. ;D

No idea, before you mentioned it, I though they were the same thing.

I think what you are talking about is the published missed approach is the one you use when ever you miss an approach. If you look at an approach plate most medium airports will have a pattern that you would fly if you miss an approach it will have you turn to a specific heading climb to a certain altitude then will vector you for another approach. 8)

Simple Missed Approach tells ATC that you are ready to set up for the approach again.  Flying the published missed approach is like garymbuska said, you are following the directions from an approach plate.  For example when I go missed, I would climb to a certain heaing and go to certain fix and hold.

In real life, you will almost always fly the published MA.  FS9 gives you the option.
