When ever i run AI smooth, it says that the sim is not running, when it is.  Is there something i have missed, as all i did was run the installer?

The is no 'installer', just the required files, a manual and installation instructions in the AISmooth.txt file.

Patched to work with FS X (requires FSUIPC 4.xx)

Have you installed FSUIPC? Link to FSUIPC 4.28

Note: you do not need to register/buy FSUIPC, simply download and install per the instructions.
