Mesh creation

Well I have a few questions on mesh...how is it made..what programs are used and does it require coding? Also can you use topographic maps to make it because I entertained the idea of making my home area a bit more realistic because there is alot of hills etc missing even with 3 types of mesh! So I tought I would give it a go. I have topo map of the whole US on this compter thar shows some great detail etc.. So I thougt if it was easy enugh I may look into it..
