Whenever a program fails, or i have to force it to shut off, my toolbar disappears, as well as my Icons. The only way to get them both back is to log off, then back on.

(the toolbar should be on top, and the icons to the left)

Looks like explorer.exe has failed. Go to task manager, file, run/new task, type in explorer.exe and hit enter. See if that happens.
